McMaster PG learners can apply for a core or elective rotation outside the City of Hamilton at the distributed campus locations of McMaster University (Burlington, Grand Erie Six Nations, Halton, Niagara, Osler, and Waterloo).
Core and Elective clinical placements within the Mac-CARE regions are requested through the MedSIS Forms Manager.
- Log-in to your FHS Forms Manager account, with your Mac ID.
- Go to Available Forms in your FHS Forms Manager menu.
- Fill out the Distributed Campus Clinical Placement Application.
- The application must be submitted at least four weeks prior to the start date.
McMaster PG learners MUST first apply for a Mac-CARE Community Clinical Placement in the FHS Forms Manager (Step 1), be approved, and have the rotation listed in your MedSIS before you can get access and apply for the commuting or housing eligibility funding form.
McMaster Postgrad learners matched to a Mac-CARE community rotation, can apply for funding.

The FHS Forms Manager will automatically calculate the distance from academic home base to rotation location and let you know if you are eligible or not.
Non Eligible Medical Learner:
- Rotations less than 4 consecutive weeks in the same community.
- Rotations less than 35 km from learners academic home base.
- Non-MOH (Ministry of Health) Funded Resident
- Physician Assistant Student
- Midwifery Student
- Clinical Fellow
- Non-McMaster Learners
McMaster PG learners MUST first apply for a Mac-CARE Community Clinical Placement in the FHS Forms Manager (Step 1), be approved, and have the rotation listed in your MedSIS before you can get access and apply for the commuting or housing eligibility funding form.
McMaster learners matched in a Ministry of Health (MOH) approved Mac-CARE community can apply for Commuting OR Temporary Housing – not both.
Rotation with location MUST be listed in MedSIS prior to funding application.
- Log-in to your FHS Forms Manager account.
- Go to Available Forms in your FHS Forms Manager menu.
- Fill out the PGME Eligibility Travel and Housing Application Form (Internal – New).
- FM PGME’s will have the FM PGME Eligibility Travel and Housing Application Form (Internal – New).
***This form must be completed before or during the rotation. Requests after rotation will not be processed/available.***
The Mac-CARE Program is funded by the Ministry of Health (MOH) to support medical students and residents from the Michael G. DeGroote School of Medicine. Mac-CARE encourages McMaster medical learners and residents to travel away from their Academic Home Base and live temporarily in the community in which they are undertaking a clinical rotation.
Mac-CARE funding is intended to offset the cost of a second residence or a daily commute to support clinical learning in a Mac-CARE community.
- Mac-CARE provides housing reimbursement up to $800.00 per 4 weeks/block, plus one return trip from your academic home base. A receipt must be provided noting payment in full.
- Mac-CARE provides travel reimbursement up to $450.00 per 4 weeks/block, inclusive of all travel-related expenses (any form of transportation, e.g. public transit, carpool or use your own vehicle). No receipts are required.

Submission & Reimbursements: Completed at end of rotation to confirm rotation details, and for those with housing funding – submit housing receipts.
McMaster learners who completed a rotation in a MOH approved Mac-CARE community, applied and approved for funding.
- Log-in to your FHS Forms Manager account.
- Go to Available Forms in your FHS Forms Manager menu.
- Fill out the PGME Travel and Housing Application Form.
- Must complete this form within 60 days of your rotation end date. After the 60-day deadline, this form will not be available, and your funding declined.
Please schedule yourself a reminder to complete this form on time.
Once McMaster learner:
- Matched to a Mac-CARE rotation, and is in MedSIS.
- Applied and approved for Mac-CARE Funding before rotation end date, in FHS Forms Manager.
- Completed the Mac-CARE rotation
- Submitted the Submission & Reimbursement before the 60-day deadline, and approved, in FHS Forms Manager.
McMaster learner will be mailed their funding cheque via Canada Post. Payment may take up to 90 days for processing.
