The visiting learner is responsible to sign up to the Faculty of Health Sciences (FHS) McMaster Forms Manager and create their account profile.

Once you create your account and profile, move immediately into Step 2.

*If you already created an account through a previous application, you can use the same account for future applications.

Learner must complete the Distributed Campus Clinical Placement Application – External Form. The application must be submitted at least eight weeks prior to the scheduled elective. If the learner fails to comply with this timeline, the elective cannot be guaranteed. The application may be submitted prior to collecting all of the required documents, however, the following items are still required to register for an elective:

1. 1. Proof of acceptance from the McMaster clinical program as outlined in step 1 (letter, email, etc.)
   2. Written permission from the applicant’s home university program director (MUST state good standing of learner, elective dates, training level, and funding source)
      Note: The funding must be in accordance with the scale of remuneration of the PARO-CAHO Agreement
   3. Proof of current respiratory mask fit testing that will cover learner during the time of the elective
   4. Proof of liability insurance with the Canadian Medical Protective Association (CMPA)
   5. Proof of licensing with the College of Physicians and Surgeons of Ontario (CPSO). For learners outside of Ontario, applicants should visit the CPSO website to begin the process at least eight weeks prior to the rotation.
