McMaster Learners must be funded by the Ministry of Health (Pertains to Postgraduate learners only)

Rotation location must be >35 KM away from Academic Home Base/Family Medicine Rural Community Matched Teaching Site

Request must be submitted either before or during rotation. Requests after rotation will not be processed.

Rotation must be in the same community for the full duration of the rotation.

Ensure your rotation details and home address, with unit number if applicable, are in MedSIS.
*Updates made in MedSIS will take 24 hours to match into FHS Forms Manager*

Temporary Housing address must be <35 KM from the rotation site
Commuting >35 KM away from Academic Home Base/Family Medicine Rural Community Matched Teaching Site

Rotation must be in a Ministry of Health approved Mac-CARE community.
